Hardship Request - Bills PRIOR TO January 1st, 2025.

If you have been directed by FPS or Kaysville Fire to submit your information for a bill consideration for dates of service 12/31/2024 and older,  please fill out this form in its entirety and allow 5-7 business days for review. Email adaniels@kaysvillefire.gov if you have any questions.
