Scholarship Review Process
1. Applicant must complete registration and turn in requested verification forms.
2. Recreation Director will review forms/verification and make a decision on if the participants may receive free or reduced recreation based on the following:
• Free activities if yearly and monthly income is below the lowest amount for household size.
• Reduced if yearly and monthly income is within the guideline chart based on household size. Reduced participant must pay the uniform fee for each sport. If multiple children from a family are participating, each family member must pay a uniform fee.
APPLICATION PROCESS FOR FREE OR REDUCED RECREATION ACTIVITIES: Our recreation offices will determine your total household size and check the income level chart included with the application form. The chart indicates the guidelines for the maximum amount to qualify for REDUCED fees. You will be notified by phone or email when the application is approved or denied; therefore, it is very important to have a correct phone number and a complete and correct email address.
INCOME VERIFICATION: Applications must include proof of financial need. This can be done by including a copy of the previous year’s tax return, recent payroll check stub etc. All forms of income must be verified including child support, unemployment, welfare, alimony disability and retirement benefits. Please attach copies of income verification to the application when applying.
*Reduced rate Income Chart: add 10% to free chart. If that qualifies you for the reduced rate you will pay the applicable uniform fee.

I understand that deliberate misrepresentation of the information may subject me to prosecution under applicable state laws. An adult must sign the application under state law before it can be approved.
